From The Fog is a Minecraft mod that adds the infamous evil spirit of Minecraft: Herobrine.
The mod brings back the fear of this entity haunting your world.
Herobrine watches you from afar when you don't expect it, and as time passes, it affects your world more and more, removing leaves from trees, making eerie tunnels underground, before finally getting to you...
From The Fog relies on psychological pressure by gradually interfering with you in increasingly indirect ways, modifying biomes, taking control of animals and monsters, and sneaking up on you. However, there may also be some face-to-face encounters.
2. Backrooms Found Footage
Backrooms Found Footage brings the Backrooms, a creepypasta about a fictional location of impossible size accessed by exiting reality, to the world of Minecraft.
The mod does a spectacular job at creating a terrifying ambient where nothing is certain; you don't know what's out there.
It comes with its own built-in shaders and controls, plus some extra features. With Simple Voice Chat installed, playing with friends becomes a completely different thing, as Skin Walkers can take their place and mimic their voices.
Backrooms Found Footage includes several levels of the Backrooms that you need to traverse safely without being caught by the entities hidden, plus a final level that is different than the usually known ones...
The first step is trying to suffocate yourself with sand or gravel to enter the Backrooms.

3. Cave Dweller
The Cave Dweller is a very aggressive mob that spawns in caves.
The entity is very tall and humanoid in form. When venturing in caves alone, pay attention to sounds and signals, as this mob has a unique spawning mechanic: It stalks the player silently and has multiple types of behavior (chase, stare, flee, stroll), and disappears if you stop looking at it for too long. Audio cues are a very strong indicator of possible danger.
Hiding is not an option, as the Dweller can climb walls, crawl through 1-block gaps, and even break down your doors.
4. The Man From The Fog
The Man From The Fog is a horror mod that primarily features showdowns with an evil creature, the Man, who hunts the player at night.
The first stage is stalking. The entity will emit a unique sound and hide to look at the player from a distance, but will disappear if spotted.
After observing the player long enough, it will strike. Lightning will hit the ground, and the Man will appear at its impact point, to then sprint towards the player at high speeds. Its damage output is very high, so be prepared if you decide to fight it.
Warning: The mod features jump scares.
If you manage to slay the Man, you'll get a rare collectible: the Tear Of The Man.
However, be aware, the nightmare isn't over yet.
5. Sanity: Renewed
Sanity adds a new indicator bar to the game. It keeps track of your mental stability, which decreases if you commit too many negative actions like murdering animals, destroying the environment, or taking damage.
The bar is shown as a brain icon on the left of your screen. When fully colored, it means it's full, and as you descend into madness, it slowly empties out until you are completely insane.
Sanity decrease is shown on screen with a red vignette at the corners and reflected in the brain icon next to the hotbar. You can also slightly recover some sanity from eating and other positive actions, and dying fully resets the bar.
If your sanity becomes too low, you will start hallucinating. Your vision will become increasingly grayed out, colors will fade, and you'll have difficulty seeing and moving.
At the final stages, creatures will transform into dark versions of themselves with white eyes and run away from you. Then, tall monsters will appear and look at you, whispering things in your ears and completely draining any sanity you have left.
Before, ultimately, killing you.
